The areas chosen as the focus for the Accelerator Programme have a clear evidence base informed by the Redbridge Public Health Report. The Partnership have selected them as there are clear overlaps between them and will require multi agency, multi disciplinary approaches to ensure effective responses. The foundation for all of them is improving multi disciplinary team working and approaches.


Mental Health

Outcomes:

  • Stigma and accessing key communities to promote mental health awareness
  • Train up wider workforce on mental health - making every contact count
  • Support the suicide prevention strategy and action plan

Childhood Immunisations

Outcomes:

  • Better and more inclusive communications, including tackling misleading information and demonstrating impact of disease (73%)
  • Training - making every contact count (clinical and non-clinical) (60%)
  • More granular data to understand groups not taking up vaccines (including in pregnancy) (47%)

Multi Disciplinary Team Working

Outcomes:

  • Support the focus on increasing mental health and wellness in PCNs and get this model right 70%
  • Focus on an MDT approach to dementia care (care homes, carers, palliative care, hospital admissions) 70%
  • Focus on children's care (esp children with asthma and LTC and link to childhood vaccinations) - can we help get better care in the community? 40%

Housing and Overcrowding

Outcomes:

  • Use of community space to support families and young people (including faith spaces) 50%
  • Better hospital discharge for vulnerably housed people 50%
  • Community engagement – understand what people want and need and what they would like us to do 50%
